In essence, we provide integrated case management. Since each case and their concerns are unique, our assessment allows us to create the appropriate team. This team will follow the agreed upon care plan so that either the family can take on what it wishes to and delegate those tasks that they may not have the time or expertise to carry out. Please contact us today and ask for a case manager to set up an assessment.
The Caregiver’s Angel™ is a step-by-step program with distinct phases to help you understand, co-ordinate and manage the care of your family member. To help you assess your situation and clarify your initial needs and future goals, we invite you to participate in our one-on-one Starter Session. When you've completed this session, you'll be ready to participate in The Caregiver’s Angel™ Program, which helps you develop effective strategies and a detailed plan. We then help you develop a team and implement your plan. The Caregiver’s Angel™ Program has a full suite of tools and capabilities that can manage all of your needs such as homecare services, retirement, co-ordination of services between physicians, specialists and government services and community agencies, as well as retirement and nursing home advice and planning.
“We developed this program in response to the rising number of individuals coping with dementia, Alzheimer's and ABI and listening closely to the needs of their caregivers who often carry a heavy burden,” says Tamhané. “We are specialists in this area and work with our clients to help them understand their current situation, overcome their roadblocks, and then develop strategies to achieve the best possible outcome for everyone.”
“As a result, you can feel confident and more at ease that your family member is enjoying the best possible care. You understand all of your options. You are getting the support you need to make the right decisions. You are managing your feelings, and are reclaiming or enhancing the relationship with your family member. Most importantly, you have the knowledge and tools to plan for your family members’ present and future care, and you have a partner to implement a strategy that is satisfactory to all parties.”
